Ligament injuries and meniscus tears are common in both athletes and aging adults. You probably known someone with a torn ACL, and you might have heard of people spraining their medial collateral ligament (MCL) or lateral collateral ligament (LCL).

Meniscus tears are common in sports that involve sudden twisting of the knees, for example football or basketball. It can also happen in older people from age related degeneration. The most common type of meniscus tears is the medial meniscus tear.

Patellar tendinitis is more commonly known as jumper’s knee. Patellar tendonitis is inflammation of the patellar tendon, which connects the kneecap to the shinbone.

Knee bursitis is inflammation of the bursa(s) of the knee. A bursa is a small fluid-filled sac that your body creates cushions the knee joint. Knee bursitis happens when repeated friction causes these fluid-filled sacs to become inflamed and painful.

Osteoarthritis is a degenerative condition affecting joint cartilage. Osteoarthritis of the knee happens when the cartilage in your knee joint breaks down, causing the bones to rub together. This happens as females have a higher Q angle than males. 

The Q-angle, or quadriceps angle, is the angle formed at the patella (kneecap) between two imaginary lines: one from the anterior superior iliac spine (ASIS) to the center of the patella, and another from the center of the patella to the tibial tuberosity (a bony prominence on the tibia). 

It’s essentially a measure of the angle at which the quadriceps muscles pull on the patella. The higher average Q-angle that females have means that the patella is pulled laterally.

In general, females have a larger Q-angle compared to males, typically around 17 degrees, while males usually have a Q-angle of around 14 degrees. This difference is primarily attributed to anatomical variations, including the broader female pelvis and shorter femur, which can lead to a larger Q-angle.

When a young female experiences puberty, their pelvis grows wider rather quickly, which in turn widens the Q angle.

Additionally, female athletes, especially those who engage in kicking, jumping, cycling and running, often report knee pain. The quadriceps are primarily active and contract when doing the above activities. When the quadriceps contract, they pull on the patella laterally and posteriorly, putting stress on the knee joint and patellar tendon.

This combined sideways and upwards pulling of the patella by the quadriceps muscles worsens knee pain for many female athletes.
