The Problem With Taking Calcium During Perimenopause

When a woman in her late 40s is told her bone density is declining, the standard advice is almost always the same: calcium supplements, vitamin D, weight-bearing exercise. Maybe HRT if the conversation goes that far.

Er Xian Tang (二仙汤) is a six-herb formula containing Epimedium (Yin Yang Huo), Curculigo (Xian Mao), Morinda root (Ba Ji Tian), Phellodendron bark (Huang Bai), Anemarrhena (Zhi Mu), and Angelica sinensis (Dang Gui). 

In traditional Chinese medicine, this formula addresses what is called kidney yin and yang deficiency occurring simultaneously — the body’s thermostat going haywire as hormone levels fluctuate and decline. It is the formula most specifically designed for the menopausal and perimenopausal pattern. 

The Science: How Er Xian Tang Works on Bone

Epimedium (Yin Yang Huo) is one of the most researched Chinese herbs for bone health. It works through virtually every major bone-building pathway: 

  • Enhances BMP/SMAD pathway activity 
  • Stimulates MAPK signalling 
  • Balances the OPG/RANKL ratio — the critical ratio that determines whether bone breakdown or bone building dominates 
  • Activates Wnt/beta-catenin signalling 

Its active compound, icariin, converts testosterone to oestrogen (the enzyme that does this is called aromatase). This is not hormone replacement — it is hormone optimisation. The formula helps the body maintain better hormonal balance during the transition period rather than simply overriding it. 

Curculigo (Xian Mao) works through a complementary mechanism — suppressing osteoclast activity while promoting osteogenic differentiation. Its active component, curculigoside, has both anti-osteoclastic and osteogenic properties, with particularly strong effects on the dual-action pathway that both inhibits bone breakdown and promotes bone formation simultaneously. 

The formula also contains Phellodendron and Anemarrhena — what TCM calls ‘yin-clearing herbs.’ These herbs reduce the inflammatory markers and chronic inflammatory state that accompanies hormonal decline. Chronic inflammation drives bone loss through multiple pathways, so addressing this inflammatory background is genuinely important. 

Crucially, research suggests Er Xian Tang has beneficial effects on bone with minimal impact on uterine tissue. The formula appears to have selective tissue effects, similar to selective oestrogen receptor modulators (SERMs), but achieved through multiple compounds rather than a single drug. This is an important safety distinction for women concerned about the risks associated with conventional HRT.
